BALLING AT THE TOWER REGISTRATION FORM

PLEASE READ FULLY and then fill out the Registration Form for Balling at the Tower 2025 to be held 1PM-3PM both on Saturday, June 28th and Sunday, June 29th at Strong Tower Ministries for AGES 14 AND UNDER. There will be a Skills & Drills Camp and 3 on 3 Basketball Tournament (Teams will be determined). 

By filling out this form, you acknowledge and consent to the following Participation Agreement, Liability Waiver, and Youth Responsibilities and Expectations:

PARTICIPATION AGREEMENT & LIABILITY WAIVER

I acknowledge that participation in the activity described above may involve risk to the participant. Risks may involve injuries including, but not limited to, the following: sickness, exposure to infectious/ communicable disease, bodily injury, emotional injury, personal injury, property damage, and financial damage.

In consideration for the opportunity to participate in the activity described above, the parent/guardian acknowledges and accepts the risk of injury associated with participation in and transportation to him from the activities. The parent/ guardian except personal financial responsibility for any injury or other loss sustained during the activity. As well as any medical treatment rendered to the participant that is authorized by the medical professional. Further the parent/ guardian releases and promises to hold harmless to activity sponsor for any injury arising directly out of the described activity.

If a dispute over this agreement or any claim for damages arises the parent/ guardian agrees to resolve the matter through a mutually acceptable alternative dispute resolution process. If the parent/ guardian and the activity sponsor cannot agree upon such a process, the dispute will be submitted to a third three-member arbitration panel for resolution in accordance with the rules of the American arbitration association.

Failure to follow the rules may result in participants’ termination from the program. 

Parent/guardian or designated person by parent or guardian is responsible for transporting participants to and from all scheduled games.

 

YOUTH RESPONSIBILITIES AND EXPECTATIONS

All players should be aware of the following responsibilities and are expected to uphold them. The rules are as follows:

Players:

  1. Berating, fighting, foul language, and swearing is not allowed
  2. Unsportsmanlike conduct of any kind is not tolerated whether during the game or afterward.
  3. No sagging!! All basketball players will dress in a respectful and appropriate manner.
  4. No use of tobacco, alcohol, and/or drugs: The use of these products is illegal and using them at any time will not be tolerated. 

Parents and Spectators:

  1. Parents and spectators are expected to show respect for one another. 
  2. Keep all cheering in a positive manner and show good sportsmanship. 
  3. Berating, fighting, foul language, and swearing is not allowed. 

Violation of these rules for a player or parents could result in a possible suspension from that particular game. Multiple game suspensions could be imposed by the league administrator, depending on the severity of the situation and offense. Continued unsportsmanlike or undesirable behavior will result in the person’s suspension from the remainder of the 3 on 3 program. Abusive behavior from fans and parents will NOT be accepted. Any spectator or parent using foul language or determined to be acting inappropriately by game administrators may be asked to leave.
